In a nutshell

Forms is one of the many digital web services part of the CitéLibre pack, which provides a form generator for any type of citizen requests, that can also capture feedback from users, conduct surveys, register for events or employing recurring services. 

The administration has worked hard to simplify the user interface as much as possible and to ensure a large-scale employment by citizens, while keeping it user-friendly.  Moreover, it is possible for the user to keep track of the received answers to its requests, to save a draft version of the form for further iterations, to upload documents supporting evidences, and finally to interact with the administration thanks to a one-way communication channel. 

The administrators can completely and individually manage the forms, schedule their publications, and test them before launching them publicly. The configuration of a form ranges from basic (i.e. collection of information using a few input fields) to very complex (i.e. conditioned display, multi-step form, uploading of documents, typed input fields, etc.). Based on the information entered by the user, the administration can process the request or ask the citizen to further complete and finalise it. 
This type of tool increases accessibility to public services through its ease of use and a 24/7 access.

What makes the service user-centric?
This service enhances the digital interaction between the users and the administration. Each user can submit a request at any moment of the day. The administrator can, depending on the level of completion of the form, directly forward the request to the person in charge of the topic in several departments of the administration, or decide to get back to the user requesting for more details to complete and finalise the form application. In this way, the digital service is fully handled online, reducing the administrative burden both for the citizens and for the administration itself.
What impact has the service had?
Forms is an open source solution developed by the city of Paris based on the <a href="https://lutece.paris.fr/lutece/" target="blank" title="Lutece framework">Lutece framework</a>. Therefore, anyone may check the codes and algorithms employed to make sure it is non-biased.
Among the most immediate and impactful benefits for the administration there is the establishment and usage of management tools allowing for an easy process of data collection thanks to the forms; a common unified interface allowing administrators to work on any typology of forms; and lastly, the possibility of a real-time follow-up of the submitted answers.

In terms of users’ benefits, the service provides the free and easy access to the platform in any moment, and from any place, ensuring a direct channel with the municipality for its citizens. In addition, it allows citizens to keep track of their requests and status.
How was the service co-created?
The development process of a project at the City of Paris involves the business and technical teams and a unit that has access to a panel of users to ensure proper adoption of the tools and an optimal user experience.

Some of the tools developed via the Lutece platform have been in place for the past 20 years, showcasing the strength of the platform and the municipality commitment to digitalise more and more services. The development team constantly relies on users' feedback in order to maintain its functionalities at the highest level and to adapt its usability according to the projects undertaken and their targeted audience, ensuring the administration to operate with top-notch level of digital services provision to its users.
